PHOTO/MEDIA RELEASE
By signing the photo release, I am allowing the use of my child’s photo image to be used in a photograph/video for the The Last Small Farm, which may include Facebook, Instagram, and other media outlets. I realize the photograph/video is for information and fundraising. The Last Small Farm or its representatives may use the photography/video images and edit them to fit the purpose of KBM. No personal information will be release about the photograph/video or disclosed in any other fashion.
HORSEMANSHIP RELEASE
This is the release and waiver of liability, assumption of risk, and indemnity. By signing the KBM horsemanship/pioneer camp release you indicate that you understand it and agree on its terms. By signing the agreement, you and your child are giving up certain legal rights, including the right to sue or recover damages in case of injury, death or property damages, for any reason, including but not limited to, the negligence of the stable, its owner, employee and agents (The “Releasees”).
I HEREBY:
1. Acknowledge that a horse or mule may, without warning or any apparent cause, buck, stumble, fall, rear, bite, kick, run, make unpredictable movements, spook, jump obstacles, step on a person’s feet, push or shove a person, saddles or bridles may loosen or break – all of which may cause the rider to fall or be jolted, resulting in serious injury or death.
2. Acknowledge that horseback riding is an inherently dangerous activity and involves risks that may cause serious injury and in some cases death, because of the unpredictable nature and irrational behavior of horses, regardless of their training and past performance.
3. Voluntarily assume the risk and danger of injury or death inherent in the use of the horse, equipment and gear provided to me by Kodiak Baptist Mission, hereinafter referred to as KBM.
4. Release, discharge and promise not to sue KBM, doing business under its own name or any other name and/or any of its owners, officers, employees and agents (hereinafter the “Releasees”), for any loss, liability, damages, or cost whatsoever arising out of or related to any loss, damage, or injury (including death) to my person or property.
5. Release the Releasees from any claim that such Releasees are or may be negligent in connection with my riding experience or ability including but not limited to training or selecting horses, maintenance, care, fit or adjustment of saddles or bridles, instruction or riding skills or leading and supervising riders.
6. I have made a free and deliberate choice to sign this Release and Waiver as a condition to Releasees allowing my child to ride or handle a horse. I have concluded that the risks involved and the release and waiver of liability is worth the pleasure of horseback riding experience.
I, the parent/guardian of this camper, hereby give my permission for my child to participate in the Youth Farm Camp at Kodiak Baptist Mission run by The Last Small Farm. I understand and agree to the following terms and conditions:
1. I am aware that my child will be consuming produce from the garden, milk from the goat and cow, as well as meat and eggs from the farm during their time at the camp.
2. I understand that all food products provided at the farm camp are processed safely and with the highest standards of hygiene to ensure the well-being of all participants.
3. I acknowledge that there may be inherent risks associated with consuming farm-fresh products, and I release The Last Small Farm and its staff from any liability in the event of any adverse reactions or consequences arising from the consumption of these products.
4. By signing the farm release signature page, I agree to assume all risks involved in farm animal activities, including horse riding. I understand that my child’s participation comes with the risk of, but not limited to, physical injury due to bites, scratches, and other unpredictable actions of animals.
5. I also understand that my child will be exposed to an active barn and have close contact with living animals, which may result in allergen exposure. As the parent/guardian, I assume all risks of my child engaging in farm activities.
6. I understand that my child may be participating in activities that are considered moderate risk. I feel that my child is developmentally ready, both physically and emotionally, and possesses the skills needed to participate in the activities. My child is in good physical condition and has not had any serious illness or surgery since his/her last health examination.
7. By signing the moderate and high-risk activity release, I understand the risk inherent in the below activities:
- Campfire cooking
- Hiking
- Horsemanship
- Wood Shop
- Blacksmithing
- Large Animal Interactions
- Fence Climbing
- Lifting 50+ Pounds
8. The purpose of the camper release form is to inform parents/guardians of the risk and to provide the opportunity for their own evaluation of the skills and behavior necessary to safely participate in the activity.
9. I authorize the camp staff to seek medical treatment for my child in case of an emergency, and I will be responsible for any related medical expenses.
10. I grant permission for my child to participate in all activities and excursions organized by the farm camp, understanding that adequate supervision will be provided at all times.
11. I agree to respect the rules and guidelines set forth by [Farm Name] and to cooperate with the staff to ensure a safe and enjoyable experience for all participants.
I have read and understood the terms of this release form and hereby give my consent for my child to participate in the Youth Farm Camp.
Anything Else We Need To Know About Your Camper?
